HARP Switches to Revised CMS-1500 Claim Forms

On Monday, February 26, Quadax will begin sending hardcopy claims from HARP on the revised CMS-1500 (08-05) claim form. The new forms will include NPIs and qualifiers for legacy provider numbers if they are stored in HARP. If you have not given your NPIs or qualifiers to us, please send them to your Quadax Service Consultant as soon as possible.
All payers will automatically be switched to the template for the revised form on 02/26/07. Some payers, however, are not yet ready to accept the new form, so they will need to be changed back to the previous version, the CMS-1500 (12-90). Confirm with your payers which form they will accept, and then notify your Quadax Service Consultants about any payers that still require the old form. Your consultant will switch the applicable payers back to the old template.
Printing claims
The template change affects demand claims that are printed to a slave printer. Demand claims, except for payers who have been switched back to the previous template, should be printed on the revised CMS-1500 (08-05) claim form. HARP will still be able to print the previous version, CMS-1500 (12-90), for payers still using the old template, so you should also maintain a supply of these forms. If information on a printed claim does not line up correctly, check to see that the appropriate claim form was loaded into printer.
If you receive claim form files from Quadax for remote printing in your office, you may begin to receive two files: one for claims using the new 1500 template and another for claims using the old 1500 template.
Conversion to the revised form
The conversion to new templates will be effective in HARP release 2.29F for all Manager and Partner clients and will occur automatically. Standalone clients will receive the template updates in a release in the near future. We strongly urge you to monitor your claims closely during this transition period.
