New HARP Forms

HARP Statement and Collection Letter Version 2 are Now Here

The wait is over; Quadax proudly announces the release of our new patient statements and collection letters. With your input, we have designed the statements and letters with many improvements and new options.

Patient Statements

Improvements

  1. A full-size (8 ½" by 11") blue and white form replaces the 8 ½"by 8 ½" green and white form.
  2. The statement body now accommodates up to 30 lines of detail, six more than the old form.
  3. The reverse side captures more detailed patient information.
  4. The statement header message has been moved to below the perforation.
  5. Payment and adjustment descriptions are indented to improve the readability.
  6. Phone numbers and office calling hours are moved to the bottom right of the statement.
  7. The message that indicates to whom checks should be made payable has also been reformatted and moved. It is located in the lower left of the statement.

New Options

  1. Address service and forwarding messages can be turned on or off for a given client account.
  2. Credit card information can now be included.
  3. Account aging information can be printed in the lower portion of the statement.
  4. Fax numbers can be printed.
  5. Units of service can be printed next to each procedure.

Collection Letters

Improvements

  • The form is now 8 ½" by 11" and the color is canary yellow for both letter one and letter two. They replace the 8 ½" by 11" white letter one and the 8 ½" by 8 ½" pink letter two.
  • The format of the top of the new collection letter is similar to the new patient statements, including a perforated return form and optional printing of insurance information.
  • The reverse side of the top of the form is identical to that of the patient statement.
  • The body of the collection letter can be up to 20 lines of a free-formatted message. Each line can have a maximum of 80 characters per line.
  • Phone numbers and office calling hours have been reformatted and moved to the bottom right of the statement.
  • The message that indicates to whom checks should be made payable has also been reformatted and moved. It is located in the lower left of the statement.

New Options

  • Address service and forwarding messages can be turned on or off for a given client account.
  • Credit card information can now be included.
  • A summary listing of all tickets can be printed.
  • Fax numbers can be printed.
  • Customizable header messages can be up to 30 characters long.
